- Black patients: 22.4% discharged with HHC referrals; 15.4% readmission rate
- White patients: 26.7% discharged with HHC referrals; 10.4% readmission rate
- Hispanic patients: 14.7% discharged with HHC referrals; 12.6% readmision rate
- Other races: 27.2% discharged with HHC referrals; 12.3% readmission rate
Notes: HHC refers to home health care referral at the time of discharge from the hospital. Results of a secondary data analysis from a multisite study of the implementation of discharge readiness assessments of 14,624 Medicare patients in 31 US hospitals from 09/15/2014 to 03/31/2017.
