- Medication errors or lack of accurate medication history.
- Medication noncompliance by the patient.
- Fall injuries.
- Lack of timely follow-up care.
- Failure to identify post-acute care needs.
- Inadequate nutrition.
- Lack of transportation to access care.
- Infection.
- Too early of hospital discharge.
- Inadequate discussion of palliative or hospice care.
