Notes: The Skilled Nursing Facility (SNF) 3-day waiver allows health care providers for certain accountable care organizations (ACOs) to admit a beneficiary to a SNF directly from the community or after only 1-2 days in a hospital, instead of at least 3 consecutive days as required by traditional Medicare.
